Accessible Format Request

Important Notice:

The Center for Disability Services will require a copy of your receipt for purchased books beforehand if you are requesting an alternate format of a published text. Please note that reproducing any digital material given by CDS that was created from published text is in violation of copyright law. By using this form you agree to abide by all copyright laws.


Below you will find the data fields for the Accessible Format Request web form. Please fill out all applicable fields and use the submit button at the bottom of the page to ensure that your form is submitted properly.

You will be notified via email that your request has been received within one business day. For this purpose, please use ONLY your AUM email address. When your request is fulfilled, a link will be sent to your AUM email that will direct you to a download through our delivery system.

You must provide CDS a copy of your receipt of purchase for each request you make. You may email a scanned copy of the receipt to [email protected] or fax it to 334-244-3907. Please fill out each field for this form to ensure that the request goes through properly. You will be notified via email within 2 business days that your request has been received.
