The Center for Disability Services will require a copy of your receipt for purchased books beforehand if you are requesting an alternate format of a published text. Please note that reproducing any digital material given by CDS that was created from published text is in violation of copyright law. By using this form you agree to abide by all copyright laws.
Below you will find the data fields for the Accessible Format Request web form. Please fill out all applicable fields and use the submit button at the bottom of the page to ensure that your form is submitted properly.
You will be notified via email that your request has been received within one business day. For this purpose, please use ONLY your AUM email address. When your request is fulfilled, a link will be sent to your AUM email that will direct you to a download through our delivery system.
